Community Case Management
Hope Association provides Community Case Management services for adults receiving Section 21 or Section 29 funding. Case management services help people with intellectual and developmental disabilities to:
- Increase independence
- Develop advocacy skills
- Connect with their community and other resources
- Help obtain and maintain housing and employment
- Provide support around goal development and daily living skills
- Maintain health and safety
- Secure and maintain benefits and financial accounts
As a Case Management Client, Hope Association can also provide Rep Payee Services.

Hope Association’s Community Case
Management (CCM) services offer:
* Representative Payee Services
* Coordinating and monitoring each persons plan to ensure individual needs and desires are met.
* Supporting physical and emotional well-being.
* Encouraging and assisting with self advocacy skills.
* Monthly contact based on the individual’s and family’s need to review goals and progress and to identify new ideas for services.
* Providing services that offer resources and assist with eliminating barriers.
* Providing resources and services to ensure goals are successfully met.
