Community Support /Days Program
Section 21, Section 29 & Private Pay
Community Support Services provide individuals person-centered services within their own homes and communities. People can choose to have their programs “within walls” or have programs “without walls”.
Community Support Services offer Individuals
- Community-based programming and recreational opportunities
- Opportunities for individuals to work on skills such as social, employment, safety, financial skills, and more
- Community activities that foster community connections and relationships
- Programs that are individualized to the needs, desires, and interests of the individual
Person Centered Outcomes
- Self awareness and self motivation
- Community awareness
- Increase and maintain physical activity
- Optimal independence
