Hope Association provides Outreach services to qualifying individuals in nursing home settings.  Criteria to receive Outreach services includes professional documentation by a qualified medical professional indicating a developmental disability prior to the age of 18 years old.  Approval to receive Outreach services is then processed through the Department of Health and Human Services.

Outreach services have proven to be an enhancement for nursing home residents who qualify for the services.  Some areas covered in Outreach services include: social and environmental awareness, sensory stimulation, fine and gross motor skills, community resources, home and community safety skills, learning and problem-solving, communications, self-care, and physical development.

Hope Association focuses on each individual’s ability and allows the individual to develop a person-centered program based on their individual strengths and abilities.  Hope Association also recognizes the  importance of music and reminiscing and includes them in daily programs for Outreach residents.
